Escalation Matrix Builder

Tiered owners, triggers and SLAs for issues that need to move from raised to resolved.

Estimated time: 4 minutes.

What you need

  • Tier structure
  • Owners by role
  • Trigger thresholds
  • Response SLAs

How to build a tiered escalation matrix

  1. Define the tiers. Set the tier structure — typically Tier 1 (team huddle) through Tier 4 (plant leadership).
  2. Assign owners by role. Owner at each tier is a role, not a name — otherwise the escalation dies when the named person is on leave.
  3. Set trigger thresholds. Define exactly when an issue escalates from tier N to tier N+1 — time since raised, severity, repeat count.
  4. Set response SLAs. Each tier gets a response SLA. An issue that breaches its SLA escalates automatically.
  5. Export and post. Export the printable matrix and post it in every tier board area. Refer to it during audits.
